Report a correction

Found a data point that looks wrong, or operate a facility and want to add context? Tell us what's off. We'll check it against the public CMS record and correct it if warranted. Because our grades are computed from public data, the fastest fix is usually a corrected federal record — but we'll always review.

Please be specific — e.g. "the fine total shown is from a penalty that was later vacated."
